SSA will gather the medical records, so you don’t have to do that. Whether you are disabled is a medical-legal issue and so Social Security doesn’t really consider a statement from a doctor that you are disabled.
But a doctor’s opinion about your abilities and limitations may be helpful. We use a checklist designed to capture that. Most doctors feel more comfortable providing that anyway.
